If you’re applying for a green card in the United States, submitting Form I-693 is a mandatory step. This form documents a medical exam and vaccination record completed by a designated Civil Surgeon, confirming that you are not inadmissible to the United States based on health reasons.
You can locate a Civil Surgeon through the USCIS website. Only doctors listed there are authorized to conduct the exam.
During the medical exam, the Civil Surgeon will:
Perform tests for communicable diseases.
Screen for certain physical or mental disorders, and drug addiction or abuse.
Ensure you have all required vaccinations.
You’ll need to bring:
Government-issued photo identification.
List of current medications.
Vaccination records, including proof of COVID-19 vaccination if applicable.
Other relevant medical records.
A current edition of Form I-693, available on the USCIS website.
You'll sign the form at the start of the exam to confirm the truthfulness of the information provided.
The cost of completing Form I-693 varies by physician, so it’s wise to compare rates. Inform the office that the appointment is for an immigration medical exam when scheduling.
A Form I-693 signed by the Civil Surgeon on or after November 1, 2023, does not expire and can be used for any future immigration benefit application. However, USCIS officers can request further evidence or a new Form I-693 if there is reason to believe the applicant’s medical condition has changed or is inaccurately reflected.
Once the exam is complete, the Civil Surgeon will sign Form I-693 and place it in a sealed envelope. It’s critical not to break the seal or open this envelope, as USCIS will only accept it if it's in its original sealed condition. The surgeon should provide you with a copy for your records.
You can submit the completed medical exam with your green card application or later in response to a Request for Evidence. Always consult with your attorney before sending anything to USCIS.
Validity of Form I-693
At the time of this writing, forms signed on or after November 1, 2023, do not expire. Those signed before this date are valid for two years from the signature date.
